If I have a heading selected in the Todo view, for example, "Red Flag", and I hit "Insert", I expect MLO to use the default new task location (Inbox, in my case) rather than some random task.  (It doesn't seem to even be the one that is selected in the Outline).

If I have a task selected in the Todo view, it properly uses that task.  But I would like to be able to put new tasks in the Inbox as well.  I really like that it picks up the flag though!!

If you select a task in ToDo and click Ins to create new task this new
task is created as next sibling for selected task in the To-Do.

To create tasks in Inbox you can use Rapid Task Entry dialog.

I'm trying to create a task with a flag, which is not yet implemented in the Rapid Task Entry dialog...it just seems like hitting Ins on a flag category should put it *somewhere* logical, not random.
