I've looked around a few times, and there seem to be some commercial
products, but nothing open source....
But not really an issue for MMD per se.

A good scapegoat is nearly as welcome as a solution to the problem.
My answer meant to imply:
1) There is no current syntax for indexing in MMD
2) I think a tool should exist that would help create an index automatically
3) In no way did I mean to imply that an automatic index would be of the
same quality as one generated by a person (or better yet, a true
professional)
The idea of indexing has come up before - it's not a feature I am likely
to add to MMD myself, but welcome ideas from others. Honestly, I would
be more inclined to devote some energy to linking to MMD to a good
indexing tool, if one existed that was free/open source. But since I
generate HTML (designed for viewing on a computer) or PDF (which I also
primarily use on a computer), it doesn't make much sense to devote time
to indexing when I can simply run a search on the full text....
The closest I have come was automatically generating an index of first
lines for a poetry book that was published through lulu.com
