When this happens, I do not get the systray icon, I do not get my
configured alert sound, and I do not get the info pop-up with the
Subject lines, etc. When it works, I get all three. No changes are
made between when it works and when it doesn't.
It will usually fail to work when doing the first mail check, when
starting the program. But sometimes it will work. It will
sporadically fail at other times during the use session.
I'm set up with four IMAP and two POP accounts. All of the IMAP and
one of the POP accounts are set with SSL/TLS. The vast majority of my
mail comes in via two of the IMAP accounts.

arrival?
Not for me, Nir. In that regard TB appears to be working
well (no alerts for Trash or Junk). As the OP noted, it is
so inconsistent that a cause and effect relationship is
impossible (for me, at least).

Similar, but not quite the same. I do have message filters set up, but
they move to other IMAP folders on the same server/account, and this
issue happens even when no messages get filtered.

Are you asking if ANY of the newly arrived mail is marked as Junk, or
are you asking of ALL of the newly arrived mail is marked as Junk?

Running Tb in safe mode + All filter and junk filtering disabled ?
Also when Tb fails to notify about new mail, do you find that new mail
in any of the mail folders by simply seeing the account icon in folder
pane? Generally when a account receives a new mail, a little red star
like icon appears on icon of that account.

I never saw such a star. When new mail arrives, I see the folder name
font change from normal to bold. Sometimes, and irrespective of the
stated problem, the folder name line will fail to indicate how many new
messages are there until I click on it - I cannot skip-to-next into it
unless it says how many are there.

Running TB in safe mode WITH all filters and junk detection ENabled,
the problem does not appear to happen. So it's apparently caused by an
add-on.
I am stingy on what add-ons I've installed, they all are there for a
reason.

Do you have any extension related to mail alert functionality installed?
Nope. The only semi-related add-on I have installed is "Sync on
arrival" to automatically get the entire message when it arrives,
because TBird doesn't on its own, despite being set to via the advanced
settings.

Tips for evil cult members: 3. Avoid needless embarrassment. Practice
the correct pronunciation of your deity's name in private before
chanting it in public. Flash cards are helpful. Be very careful to
pronounce only one syllable at a time; some deities pop up at every
mention of their name, and expect to have an acceptable sacrifice
waiting for them.
