Revised Commit Access Policy now official

The revised policy:

1. specifies the repositories to which this policy applies
(mozilla-central, comm-central and the main CVS repository)

2. the "General Rule" (a) removes the "not your employer"
requirement; and (b) changes SR criteria by removing "different module"
language and replacing with "must not have previously reviewed or
super-reviewed your patches" and adds rationale

3. "Modules Not Associated with . . . " This has changed to be clear
that Calendar will now need SR OK for commit access, and then
Calendar-focused contributors are full-fledged contributors to comm-central.

4. Revoking Accounts section added

5. placeholder Dormant Accounts section added, noting we're working on
this topic, looking for a lightweight way to address non-active accounts.
