Submission to DEJUS?

While waiting for approval, I downloaded the [DEJUS form][1] to apply
for L rating in Brazil.

Are there any suggestions concerning what values to provide for a
Firefox OS app? Can I even submit the form as a non-Brazilian citizen?
Perhaps this could be added to a wiki page?

I wonder about the following general entries, and especially about the
address:

* Platform:

+ Cell / Smartphones

+ Other: Web

* Distribution Media:

+ Internet Link

* Game publisher: <my name>

* Country: <what's printed in my passport>

* Game distributor in Brazil: Mozilla Corporation

* Media sent to the Ministry with the content for rating: Internet

* Rating Requester: <looks like this has to be the address of someone
in Brazil - what to do?>
