We use Places to query the most frequently visited pages. There is some

That's a good question. It probably does not have a high frecency score
because you never type it and it's "only" your homepage. I don't know
much about the frecency algorithm itself but there are people that
hopefully can explain this better.

Thumbnails are taken while you're browsing so that they show a realistic
image of the page. They do not reflect the current state.
