In 2010 I converted from a deductible IRA to a ROTH IRA, I opted for 2 year treatment of reporting in 2011 and 2012. My question is do I use original 1099R and use 50% of it or what? 8606 appears unwilling to help?

Yes. Half was reported on the 2011 return and the other half reported on
the 2012 return.

If you're preparing a paper return just add the 2012 amount to line 15b of the 1040. No other entries, including the 8606, are required.

This sounds like a software question. Did you import the data from your 2011 return? This amount appeared automatically for me. If you can't figure out how to get the amount to appear, check with software support. I would not fake a 1099-R since this might leave IRS computers looking for more income.

If you are using TurboTax, the 2010 contribution goes on the IRA Information Worksheet, Part V, lines 22 and 23. For me, it automatically flowed there when I imported last year's information. If you are using another tax package, I suppose there is a similar place for you to put it.
