It is state law here in MN that an eye doctor has to give
you a written prescription that you can have filled anywhere.
I believe that most states have that law these days.

Of course, a good eye exam should be more than a simple prescription.
It's usually best to have one through an ophthalmologist (doctor) vs. an
optometrist.

Not necessarily. Ophthalmologists are doctors specializing in eye diseases and
only secondarily provide refractions. Optometrists can also deal with the more
common eye diseases -- if they see something they can't deal with they send you
to an ophthalmologist.
Call Walmart and ask what they charge.

That's my opinion. Ought to be yours.
I don't know anything about Zenni but for a quick answer, it costed $70 for
an eye exam and prescription. This is at a Walmart in South Carolina.

Each spring for the past several years, Sears has had a 50% all lens /
frames promotion which I've found to be very competitive - especially if
you want / need a loaded pair (ultra low weight, transitions,
progressive bifocals, etc). They have also honored that promotion for
Verilux which are typically more costly than other knockoffs, but tend
to have a improved / wider viewing range & reduced distortion (some find
the improvements to be marginal, others significant).
For a basic lens / frames, it'll be hard to be the china-imported
components via Walmart.
Except by going somewhere like http://zennioptical.com, which in fact
is what the original poster said they wanted to do.
(I haven't used zenni myself, but I've got a friend who has with good
results.)

Yep that's what I want to do
