Breaker Box

I've got a friend whose home is in need of rewiring.

There is a 20 slot Siemens breaker box with QP breakers, circa 2003.
But there is no main breaker, Service wires go directly to the bus bars.
Outside at meter, there is a 150 A breaker, attached to the meter box.
Is this right?

From what I've seen, yes. I'm not an electrician, though--but I have
seen that configuration installed on an episode of This Old House.

thats actually a better design, a failure past the meter but before
the main panel is protected. and working in the main panel is safe....
