well first thing i have to ask is do you have any mods downloaded?

if so then some of your mods are conflicting, if that's not it then i'm not sure what it could be. sorry that i couldn't be much help.

simple....delete the META.INF file. thats causing your problems, also force update and make sure you dont have a lot of other programs that may take up your pcs performance.

the maybe you might be using the actual minecraft.jar, you must use the mineshafter in order for it to work. ALSO you must have a server from mineshafters SERVER LIST. not just any premium actual minecraft server.
