WSUS problems on disconnected network

I have a disconnected network WSUS 3.0 SP1 installed. My issue is that I
cannot get the "Updates needing files" to display anything consistent. I have
downloaded ~49 GB from the external server to the disconnected but my current
status is
Updates needing files: 201
Downloaded 417.81 MB of 6771.24MB
My wsusutil import and export both seem to work OK.
I have ensured that the language and product settings are the same on both
sides. I have declined all the updates on the inside that are declined
externally.
I have set my syncronisation to "manual" on the assumption that no
syncronisation is possible - setting it to "microsoft updates" returns an
error (to be expected).
My WSUS updates folder has > 100 GB free and the permissions are OK
Can anyone point me to something I've missed?
My clients are all registering OK and the console is working well but I'm at
a loss on how to proceed. Most of the updates are displaying "Waiting for
download to complete'
