Problem with Custom Content Type and Document Properties

Hi, all. I'm unable to successfully add a CustomContentType to a Document
Library and have the Document Library Properties/Columns show as Properties
in a Word 2007 document. Here's the situation:
1. Created a CustomContentType containing 4 additional columns.
ParentContentType = Document. Uploaded a Word 2007 DocumentTemplate (File
extension = .dotm) as the template for this content type because of a macro
in the template.
2. Created a new Document Library. Checked the "Allow Management of
Content Types" box and added the CustomContentType to the DocLibrary.
3. All columns from the CustomContentType appear in the DocumentLibrary AND
I can choose them to be part of the Library's default view.
4. Create a New document, choosing the CustomContentType instead of the
regular Document type.
5. In Word when I click Prepare, and Properties, the ONLY property shown is
the default Sharepoint "Title". NONE of the other columns appear in the list
of "Document Properties - Server".

WHAT HAVE I MISSED? I saw a similar post commenting that the problem was
because the ParentContentType for my CustomContent = Document but what other
choice is there? Do I need to create a CustomLibraryTemplate too?

I am currently using a MOSS environment but have experienced the same
problem in a WSS environment. AND I have gotten this to work correctly in a
different site before but that was after I had gone through mutliple
iterations of creating CustomLibraryTemplates (before I figured out that
those weren't going to do what I needed to do and switched to
CustomContents). So my thought is that maybe something I did there is the
missing step even though I can't see anything having to do with the custom
libraries in the CustomContents that are working.
