The album art needs to be physically present in the form of a .jpg file on
the PC. If you look at folders of albums which have album art automatically
added to them, you'll find something like this:

AlbumArtSmall.jpg
AlbumArt_{9E8480EC-517D-48B5-8681-D4B302A67EEA}_Large.jpg
AlbumArt_{9E8480EC-517D-48B5-8681-D4B302A67EEA}_Small.jpg
Folder.jpg

Folder.jpg is the one you need to add manually.
