Problem adding XSLT as Data Connection

Hello,

I'm folling the instructions to use XSLT and C# to convert Infopath 2007
form to Word by S.Y.M. Wong-A-Ton (posted here

and when I do steps to add the Data Connection (steps 9-17) I get the
following error when running the form:

Some rules were not applied.
Unspecified error

Then, my form gets corrupted as even when I remove the new Data Connection I
keep getting the same error. How do I troubleshoot this problem?
