I finally picked up KEY the Metal Idol volume 1 on DVD,
and watching it in Japanese, is like watching the series all over again.

Anywho - my question is: to those of you who have the vol. 1 DVD,
does it have that little slip of paper with the chapter names and stuff, or
were those only in Vol.2 & 3?

It does come with the slip, sometimes anime dvd's will
accidentally leave the slip out. As for the box set,
I'm pretty sure they're the same dvd's, but it
includes the soundtrack as well.
