Defects on Clad Weld Overlay Material Inco alloy 825

Dear all 

I witnessed DPT on incolloy 825 Weld Overlay and found some defects Like LOF, Porosity and undercuts, Defects were repaired by Subcontractor and offered and found acceptable but after one year again DPT carried out and found all the defects like porosity, LOF etc.what may be the reason.

Is it possible that Subcontractor used wrong welding procedure to carry out the repair and after some period defects gets open.

Pls convay your ideas for the above.
