Looking for a plugin to insert a registration form into a page

Hi,

I'm building this website (using synthetic theme):

I want to people to be able to sign up in three different ways (see further info below):

a) As a contributor  (see how I've done this below)

b) As an author

c) As a newsletter subscriber  (see how I've done this below)

Is there a sign up form plugin I can use so that new users can select author as the role, and then the admin will approve this?

I've seen:

Simplr User Registration Form - but it seems to have broken some people's websites so a bit wary (also not tested up to 3.4) Anyone used it?

a) As a contributor - to comment on posts - I've put a join button on this page:

b) As an author - I'd like to have a form to automate this sign up that the admin can then approve

c) As a newsletter subscriber - I'm using the madmimi newsletter plugin - so that's sorted:
