MD5 assembly code ported for Microsoft ML64 for AMD64

I've ported the Marc Bevand work for MD5 on AMD64 from GCC/GAS to Microsoft ML64

I made a md5-ms-amd64.pl from md5-amd64.pl, which produce ML64 (with Intel/AMD mnemonic) instead GCC/Gas file.

I'm using OpenSSL5 only for MD5, I use only 1% of the library, but it is very good ! :-))
I think openSSl provide the faster MD5 we can found on the internet.
