cant create or edit innodb tables after upgrade

I recently (a few days ago) upgraded from 4.1 to 5 (mysql Ver 14.12
Distrib 5.0.19, for pc-linux-gnu (i686) using EditLine wrapper).

Now anytime I try to edit or create a table to be InnoDB I get errorno:
121

mysql> create table numbers ( numberid varchar(10) not null, number
varchar(10) );
Query OK, 0 rows affected (0.09 sec)

mysql> alter table numbers type = InnoDB;
ERROR 1005 (HY000): Can't create table './switchboard/#sql-4f6c_b.frm'
(errno: 121)

Even if I create a new blank database and new blank table - I get the
same error.

I dont see anything amiss (although I may not know what to look for).

Regular query operations are working fine - but creating InnoDB tables
or changing a table to be InnoDB always fails.

Any Ideas?
