They are not required fields. It would be better to just leave them
blank. Blank info won't do any harm. Bogus info will just give a
false representation of the group's makeup.
City is a required field though. It is so we can get an idea of where
people need to drive from.

Ahhh, it didn't indicate that. Initially I only filled in name, zip
and email and it spouted an error asking to fill in the required
fields, so I thought that everything was required
I've filled it out with my correct name, zip and email, and I answered the survey to the best of my ability. Sorry for the bad data (address and phone) :-(
