Ahhh, it didn't indicate that. Initially I only filled in name, zip and
email and it spouted an error asking to fill in the required fields, so
I thought that /everything/ was required
I've filled it out with my correct name, zip and email, and I answered
the survey to the best of my ability. Sorry for the bad data (address
and phone) :-(
