Gaming Collection Survey for Public

I am trying to come up with a survey to give the public in order to
figure out what equipment the majority of the population uses and what
games they are interested in, but I am at the end of the yeard brain!
Would you anyone have a sample survey that I may use as a starting
point. You can email the documents to my gmail account.

Hi Angela,

We use an online survey source:  www.surveymonkey.com.  It's free if you have under 100 respondents, or you can pay for a subscription with more features.
