Can someone post a sample forms submission?  Preferably something with a couple of fields with the header/footer and a submit button that I can use within Kurogo for a feedback content module?

Bump!  Any easy examples someone could help me with?  Just want a way someone can enter in some text, push a submit button and have it emailed.
