Placement of Headstones

I am looking for reference material with regard to the standards when it comes to the placement of headstones in a Jewish cemetery - meaning whether the deceased is
buried in front or behind the stone.

Furthermore, is this something based on tradition or regional customs?

Once again, references are important.
