I doubt if this is related to the dongle, but maybe I'm missing something. Sounds like a power problem. Is your power supply OK?

Have I don't something wring and is their anyway I can get the IOIO working again (reset, test if it is broken etc)

1. The power has not changed. Literally as soon as I plugged the USB in the light went out, now the power LED will not come on under any circumstances except 2 below
 
2. But, the device does power when connected to the PC in HOST mode (i.e. I can power it from the laptop's USB power).
 
3. The current status is that I can use it connected to a PC using SWING, but cant get anywhere connected to the Android device or in isolation.
 
Maybe, when I connected the Bluetooth it got a static surge (stupidly it was connected to the power when I inserted the USB dongle)
 
I have another board on its way, although I am bit nervous plugging the Bluetooth dongle in until I can figure out the cause of the first issue?

Your VREG seems to be fried. Pluggin / unplugging USB devices while the IOIO is running is perfectly OK.
I can't see how your dongle could have caused that.
If you have a voltmeter, can you see what the voltage on the Vin and 5V rails is while powering the IOIO without anything else plugged?

The VIN is 12v (it is a 12v supply), but the 5v rail is barely registering 0.1v

Dead VREG. Not many things can kill it that I'm currently aware of, save reverse input, which is not what you had since the board has been working until you plugged in the dongle or possibly ESD.
I think it is probably safe to try your dongle with the new board, but of course can't be 100% sure since we don't know what caused this.

This has happened again on a  new board!! This time I heard a bang and spark from the board and then a slight smell of burning
 
I actually got the Bluetooth to pair and it was working, this only happened when I plugged it in another time.
 
Same behaviour as before, all power seems to be working but the board is not recognised by anything and now the Bluetooth is not visible (even though the Bluetooth dongle LED is permanently on)
 
could it be the Bluetooth dongle? Any ideas, I don't really want to blow a 3rd board.

I am a fool! I re-looked at this and have blown the chip in the middle by accidently loading 15v to a 5v port.
