GPL license is missing in some PHP files

I've found, that we haven't added GPL license code in some of newly added files.

I think, that we need to create some script, that will check license presence in all PHP files.

I think it's a good idea to have such script.

Before we do that can you list the files which you found that missing
license info?

Here they are:

• email_event_tp.php
• form_field_eh.php
• menu_helper.php (only "svn:externals" property missing)

Patch for task about missing GPL license (924).

Alex, please review and commit changes if satisfied. Please note that haven't done any changes related to missing EXTERNAL for menu_helper.php
