Intracare Implementation Log Episode 25: Demystifying Templating, Document Classes and Titles for Dummies.

Once you have a system up and running, a large part of time (at
Intracare anyway) is spent getting the forms into templates, managing
templates, and getting the document dialog boxes to display custom
titles and flow well. This is the real pay-dirt of the system. I hope
to demystify the process with just what you need to know and nothing
you don't. The terminal sessions below I believe covers about 99% of
what most people need to know. It creates a Progress Note TEST1
document class and a TEST1TL title class, makes them active and then
shows how to add template text into the Boilerplate field to make it
automatically pop up a template associated with the TEST1TL title when
a New Progress Note... named TEST1TL is added.

So in this episode, on a WorldVistA EHR/VOE 1.0 server we will:

1) Create a Progress Note Properties dialog box Progress Note Title.
a) TIU IRM MAINTENANCE MENU--> 2 Document Definitions (Manager)
2) Make it active.
a) TIU IRM MAINTENANCE MENU--> 2 Document Definitions (Manager)
b) Define VHA ENTERPRISE STANDARD TITLE
3) Make a Progress Note Title automatically pop-up a template in CPRS.
a) TIU IRM MAINTENANCE MENU--> 2 Document Definitions (Manager)
b) Add template text to Boilerplate field.
4) How to make a template Shared.
a) Be a Clinical Coordinator.
b) Fire up CPRS-->Notes-->Options the rest is self-explanatory.
5) Fixing Location for clinical user.
a) Select New Patient-->Click location-->Save Patient List Settings.

Take home message is that the TIU IRM MAINTENANCE MENU--> 2
Document Definitions (Manager) ... is your friend. There are loads and
loads of TIU menus but this is the key one for managing the CPRS
Progress Note dialog boxes. TIU DOCUMENT DEFINITION file is the one
all this occurs in. A-ha for making a title Active from Inactive is
defining its VHA ENTERPRISE STANDARD TITLE which is a huge list of
possibilities, fortunately it seems to cover nearly every case. F1
pause then e will save and exit from the boilerplate editing session.

The terminal sessions below are the basic but very important maneuvers
that really make the VistA system work locally. Try not to stray from
this recipe or these menus as you can really foul things up (like I
did) and have to call in experts to do arcane maneuvers like delete
orphans which may bork your system for a day (like I did).

While this example does not use Kevin Toppenberg's GUI-Config on
purpose, once again, I cannot say how great Kevin Toppenberg's
GUI-Config utility is. It dramatically simplifies doing and
understanding some things, in this case being able to look around
broadly at these files and fields as well as making defining the
Boilerplate once the title entry is create a snap.

You usually make a template in the GUI and assign to "Document Titles"
part of the tree in your Template editor. Once you choose that
specific note title, you will get the template. That's much easier
than going in roll-and-scroll and doing it there.

cool! Does this define a new note title for everyone? Why aren't
current definitions in that folder?

Oh yes, Kevin. Once you attach a template to a note title, anybody can
access it by selecting that note. You can make it a default note by
going to Tools>Options whenever you write a note.

You sill have to define the note title in TIU Manager Menu on the roll
and scroll side.

Yes. If you attach a note title to a template, anybody with access to
CPRS when selecting a note title will have this template pop up.

Okay, I've played with it some more. As far as I can tell you can
associate it with an already existing note title but it does not seem
to create new note titles. If you want custom titles to associate with
then you still have to go into text based and define a new title, is

get the expandable/collapsible parts into Boilerplate so that it comes
up automatically when the document type is selected? Right now I'm
seeing that Boilerplate looks flat and unable to support

Let me expand a bit on Ignacio's question, after talking to him today.

There are at least two ways of getting templates into a document in
CPRS. The first is by a template. There are examples of templates
from the VA etc that are multi-level. That is to say, that when you
check a box, then the template will expand and give the users more
questions to ask. When you go into the template editor, these are
organized in a hierarchical manner, looking almost like directories
and sub-directories.

Eddie made such a template for Ignacio. Ignacio had a questionnaire
for nurses to fill out that included a long section about
adolescents. He didn't want his staff to have to deal with that part
for adults. So Eddie followed the example of the VA templates and put
that part in a sub-document (I think the term is "drawer"), and now
when this template is launched, there is a checkbox for Adolescents.
If that is checked, then the questionnaire is expanded as expected.
And if not checked, it is skipped.

Now on to the second way of getting automatic text into a note. And
that is with a boilerplate. This is a template that is launched
automatically whenever a particular title is selected. If this
boilerplate text includes |TextObjects| then the objects will
evaluated. For example, |PATIENT NAME| would be turned to the proper
name etc.

So Ignacio has copied the template, which works as a template, and
pasted it into the boilerplate field for the document definition.
Now, when he launches the particular note title, the boilerplate gets
executed, and he gets all the dialogs etc that he expected. EXCEPT
that the Adolescent checkbox to expand is missing.

So here is the question. Is it possible to have sub-documents, or
drawers, or whatever they are called, when using boilerplate text?
