Habari 0.8: When are comments silently rejected?

My friends just told me that their attempts to post to my Habari 0.8
site have been silently failing. (The page seemingly reloads with
form data populated with what they just submitted, without the
expected notice that the "comment is pending moderation".)

The failed comments are not marked as spam. When looking at the
Comments admin panel, filtering to show *everything* the failed
comments still don't show up at all.

I can reproduce their experience sometimes - when in Chrome Incognito
mode, and from Safari on a Macintosh. Otherwise, commenting works as
expected, they go into moderation, and I get email notification.

I don't know anything about HTTP or PHP, so I'll provide this info:

I've turned off all plugins except Core Dash Modules 1.1, Flickr Media
Silo 1.0, Habari Media Silo 1.0.1 and Undelete 1.0.
I've set logging to any and debug, but neither one logs anything when
comments don't successfully get recorded. On that note, there are no
suspicious warnings or errors in the logs after the comment failures.

I did customize the comment form like so (adding a txt exension to a
copy of the actual files so that you can see them):

Three questions:

1. Does anybody know why sometimes commenting succeeds, and why it
sometimes silently fails (when it's not marked as spam)? (Please feel
free to experiment at my site. I'll eventually delete successful
comments that are only for debugging purposes.)
2. Is there a way to notify the user why their attempt to comment
failed?
3. Setting Admin->Options->Publishing->Comment SPAM Threshold to 100
percent is not understood by me. What does that number actually do?

Follow-up: Two days later, and I cannot reproduce the
silently rejected attempts to comment. So there's nothing to
investigate,
except it's a little worrisome that whatever trouble there was, it
didn't
tell the user there was a problem nor leave a log.
