Is there a specific time frame or number of times that it takes for
Google to remove the page from the index?

Like, if Googlebot tries to access a page once and can't get there.
Does Google remove it after the first time they try to access it?  Or
is it a three strike and your out rule?

How long would it typically take to get back in the index
automatically?

Is there an index for 404 pages?
Is there an index for pages that time out?
