I have an application that's wholly GWT (and EXTJs) based, I have
Chrome Frame installed.  If I connect to the app via http:.... then it
runs 5 times slower than if I connect via gcf:http:....

There's no compatibility icon in the IE explorer bar and I set the
meta for IE8:

<meta http-equiv="X-UA-Compatible" content="IE=8"/>

I'm connecting to an internal site and the "display intranet sites in
compatibility view" checkbox is set and disabled.

RTFM - Only the first X-UA-Compatible is processed, I was creating 2
of these (one for IE and one for chromeframe).  Solved the issue by
adding chromeframe=1 to the content:

<meta http-equiv="X-UA-Compatible" content="IE=8; chromeframe=1"/>
