ads stopped showing after being fine for several days

I implemented admob for a new iOS app, the app launched about 2 weeks ago. 

At first I was seeing ads, no problem. Looking at my "user activity report", I see that there was activity from 5/31/2022 - 6/8/2022. Both columns, "Daily active users" and "Daily ad viewers", have some number in them.

But starting 6/9/2022, only DAU has numbers, the DAV column shows 0.

In the code, the error I get back is below. So it implies that for some reason the ad flow has been turned off. I haven't done the PIN thing, since I haven't crossed the payment threshold (very little traffic so far). Nothing shows up in "Policy", I haven't received any emails, I did implement an app-ads.txt file which has been successfully crawled.

So I'm stumped. For 9 days things seemed fine, and the last 6 days....nothing.

Further, the customer support "link to email" is broken, I keep getting an error: "Something went wrong, please try again", and it's been broken like this for several days.

It just feels like something got triggered that shut off the ad flow, but have no idea what that could be. Is there any code I can add that might give more info as to what the issue might be? Any ideas as to the problem?
