No insurance and preferably under $100. Most likely a pipe dream but
It doesn't hurt to ask.

My last one was at VisionWorld. $39 on a coupon special. They have ads
for eye exams in the Sunday paper almost every week for $49.

Check yelp in your area. Costco, cohensfashionoptical.com. Do any
schools do this cheap? In most cases you may need to be a bit
assertive to get your prescription written out. Step thru the
ordering form on one or two sites before you go so you know what they
look like and then when you get the prescription you can glance at it
and see if you comprehend it. They seem to be sloppy about the minus/
plus signs, and decimal points. Ask if they can measure your PD
(pupillary distance) for you. Some will, but often they won't - it
crosses some kind of professional boundary. If you have bifocals
there maybe two of these. I was able to get three prescriptions last
time I went - bifocals, reading, and computer; he was happy to do it.
Finally if you have health insurance it may include the exam, it
appears to be required here in Massachusetts.
