Resolution was to perform the following mysql statements:

select group_id from account_groups order by group_id;
alter table account_group_id auto_increment=nn;

I've just installed 2.5.4 from scratch (new site, new db, ...) leaving the
apache settings untouched. It worked.

Yes, a fix for this was done in gerrit-2.6-rc4.war. This is why this issue
