When I install firebug the icon shows in the toolbar. When I restart FF, the icon is gone. Yes, I just created a new profile, nothing but firebug.
When = I install firebug the icon shows in the toolbar.

Oh, it looks like I can still access i= t from the right click menu, but I can seem to put the icon back in the bro= wser toolbar...
Did you try it via the Customize Toolbar dialog?

When the icon is missing, it is not available in Customize= either. It behaves as though FB is not installed.
