I have the same problem on Firefox 14.0.1

Hm, actually I have to disable/enable Firebug now each time I start the 
browser to make it working again...

Yes, I just created a new profile, nothing but firebug.
When I install firebug the icon shows in the toolbar.
When I restart FF, the icon is gone.
When I disable/re-enable FB the icon comes back.

When the icon is missing, it is not available in Customize either. It 
behaves as though FB is not installed.
