hard to track down error with comment widget

I'm having an intermittent problem with the comment widget, where you type in a new comment, hit the post button, the widget acts like it normally does and does a quick refresh flash, but the comment doesnt show up in the stream. no client side errors, no server side errors. and if you refresh and try again later, no problem either, it works as expected, on the exact same url. I'm fishing in hopes maybe someone has come across this and can save me some debuggin-
