Frequently I'll have two instances of Safari.app in my dock and two
instances in my Recent Items list. I have checked and I have only a
single copy of Safari.app in my Applicatioins folder. My understanding
is that one should only have one instance of Safari.app running, or am
I mistaken?

You can set your computer to use to instances of an application. I do this sometimes, as and when I need to!

I discovered this on this website : http://osxdaily.com/2011/05/11/multiple-instances-application-mac/

I hope this helps,
