Subdomain naming best practice recommendation

I wonder if anyone has suggestions to how to name subdomains for a
medium-sized IT/IS service company with approx 1000 employees in 20
sites, 600 servers, 1000 network devices. Should we use a flat naming
scheme or a hierarchy based on locations or organizational structure ?

Should naming governance be established or is controlled anarchy the way
to go ?
Any experience with amount of work when organisational structure changes
etc. is most welcome.
