Real World Value LUT

Hi All,
I'm not sure to have a good understanding of how to use real world value LUT, especialy with or versus Modality LUT. As far I understand, modality LUT is here to transform "proprietary" stored value to standard modality value and it is used in the display pipeline, ok. Real world value LUT transform stored value to a unit.
What are we supposed to understand when both are present (if it is possible?). Real world value LUT is to be applied to all values given to the image reader or it should be used as an alternative unit that can be given in addition to the modality one?

Well, I'm a bit lost, if someone could help me or giving some pointer where I could find more explanations it would be very helpful for me.
