Maybe you already have found it. Anyway, modality tag is (0008,0060)
Here you have a list of modalities:

You could find additional information in the "SOP Class UID" field (0008,0016)

Yes the below tags can give this information:
(0008,1090) Manafacturer's Model Name
(0018,1020) Software Version(s)
(0008,0060) Modality
