Why no standard for abdominal Ultrasound in DICOM SR - only ob and vascular

Does anyone know why there is no standard for DICOM SR for abdominal
reporting? There is one for OBGYN and one for Vascular. I'm puzzled
as to why there's not one for Abdominal. Radiologists are stuck
dictating these numbers instead of having an IT solution to help.

Because nobody has seen fit to put in the time and effort
to write such a template ?

Feel free if you want to.

How standardized is such a report amongst different users ?

In lieu of a standard Template, vendors can also implement a private
SR Template to convey whatever measurements they collect in their
applications. It may be that the abdominal ultrasound applications,
especially in newer versions, are already capable of outputting such a
private template SR. You should check your vendor's DICOM Conformance
Statement.

However, I need to say that private SR templates are invariable of
poor quality. Defining a proper SR, with appropriate use of coded
vocabulary and hierarchical Content Item structures, is extremely
difficult. Even the SR Templates brought for standardization require
many iterations before they are finalized. For example, Pediatric
Echo Supplement 78, now being released for Ballot, has been through
over 40 revisions. SR definition requires skills in conceptual
modeling that and deep understanding of terminology systems that are
not broadly developed in the teams that typically implement image
analysis packages. Having an SR Template go through the DICOM
standardization process means that it will receive an in-depth review
and generally be of much higher quality.

If you know radiologists interested in an abdominal US SR template,
please contact me, and we can investigate preparing a DICOM Work Item
to address this issue.

Thanks David for your quick response.
I have another question - are the Dicom file and the dicom sr coming in different files or its one file contain everything.
i want to make my question more clear.
when the radiology make ultrasound scan for pregnant women he send the file from the ultrasound machine to the computer thru dicom connection or any other way.
the file he send is one dicom file that contain the images and the fetal measurements together or they come in one file.
I am asking you this question because today we are making all the process manually and we want to automated it , like send the file to the computer that have listener and than we can collect the data i mention above.
can you tell me how we can do it or at least tell me when i can learn about it more.

Hi David,

TID 2000 Basic Diagnostic Imaging Report seems to be a generic template in which any measurements can be included.
What are your thought about recording ultrasound abdominal measurement in a SR based on TID 2000. Measurements' EV can certainly be taken from LOINC or SNOMED database.
It is certainly better than recording no measurement at all and would reduce the risk of error.

DCMTK (www.dcmtk.org) is an open-source DICOM toolkit that has a module
specifically supporting the reading, writing and manipulation of DICOM SR objects.

Hello David

I have question about ob-gyn dicom file
when the dicom file sent from the ultrasound machine
is it include all the information about the fetal biometric fetal
like HL AC measurements and all the parameters.
how i can get this information from the ultrasound machine.
does the dcmtk has the ability to get it.
i am looking for dicom file that contain this information
