This has been my conclusion as well.

The problem with an index file is that such a thing is not part of
maildir, and if the index file is not NFS-safe by maildir's standards then
it violates maildir.

Unfortunately, it does not seem to be possible to have a reasoned
technical discussion about this issues with members of the Church of
Maildir.  This is particularly troublesome for me, since I'm totally
dedicated to the Church of IMAP :-) and any technical solution that I
endorse must be fully compliant with IMAP.

Other people are willing to cut corners with IMAP and/or maildir, and then
turn around and claim that there is no problem.  The Courier server is a
good example of this; it flagrantly violates IMAP in multiple ways and I
think that it also violates maildir.  You can watch the ongoing
performance of Courier's author on comp.mail.misc to see why I choose not
to communicate with him.
