You can't span multible pages with a single form. Call the Submt button on
each page's form "Next Page" and pass the form data through the
form-processing CGI tto the next form, creating the form on the fly and
accumulating the data in hidden fields. The final Submit on the final page
of the multible page series would send all the data in for final processing.
