Lightroom Metadata plugin for "16 free form fields"?

I have been using a Lightroom 3 plugin that gave me 16 free-form
metadata fields. I have tens of thousands of photos with info in those
fields.
I installed Lightroom 4 today, and that plugin is gone. I went back to
LR3, and it is gone there too?
Worst part is I can't remember the name of the plugin, and thus have not
had any luck finding it on my disk or on Google to replace.
If I remember right, I paid something like $5 or $10 for it a couple
years ago.
Anybody know the name (and/or where to get it)?
