Harold Lohner's JJ Stencil could probably be used to create such as
that:

www.whatthefont.com should be able to help you for future items, but you'd
need to submit a smaller file - not so much in filesize, but in the amount
of text to read, it has a limited amount it can read.

It's definitely Harold Lohner's JJ Stencil. I e-mailed the sample to
him and he confirmed that it is.
