What questions would you ask CIOs/CSOs on the topics of SaaS?

Hi all,

I am working on a list of questions that can be used as a short CIO/CSO questionnaire. I am thinking no more than ten questions as their time is valuable and when the questionnaire gets too long, they will more likely ignore it. I will be posting, with permission of course, their responses on OnSaaS.net as I get them. The goal of the survey is to get their views on topics such as 

- the current SaaS adoption of the company (what type of SaaS products have been adopted, if comfortable of sharing publicly, which SaaS products are used)
- current experience with SaaS products (security/trust, integration, customization)
- future expansion/reduction of SaaS adoption (what other areas will likely get SaaS'fied or brought back in house)
- selection criteria (what are the top 3-5 things to consider when selecting a SaaS product)
- certifications (what certifications they are looking to see, e.g., SAS112, SAS70, PCI DSS, etc)

Given that the answers will be posted publicly, depending on the level of comfort, we may or may not post specific products they are using. We may also hide the actual name and company, again, depending on their level of comfort.

So my question to the group is, if you have an opportunity to ask enterprise CIOs/CSOs 1-3 questions on the topic of SaaS, what would they be and why?

Maybe something like, What are your concerns for using cloud computing
in the enterprise?

Or maybe something more blunt like;
Have you ever heard of Cloud Computing? If so, can you define it?

I would want to ask:

What challenges are they currently facing in the on-premise model?
What considerations would they like to see solved (or answered) for
them to adopt the SaaS model? (Things like security, availability,
etc.)
And maybe to tweak Reuven's question -- how familiar are you with SaaS
and/or cloud computing? (multiple choice answers)

Oh, you might want to check out surveymonkey.com too -- it's a tool we
use to build surveys.

What about an open question on the kinds of areas where they see SaaS
being of most value compared with other things?
