Error handling proposal

I have begun to document what I hope will be become a comprehensive enhancement to Clojure's error handling at http://dev.clojure.org/display/design/Error+Handling.

If this works, it would subsume and/or supersede the following:

* all the error handling and condition code in contrib
* all third party stacktrace helpers a la clj-stacktrace
* all previous Confluence pages re: errors, stacktraces, assertions, etc.
* all oustanding JIRA tickets re: errors, stacktraces, assertions, etc.

Definitely a work in progress! Pleas use the comments feature in Confluence to add your thoughts.
