To All,

 

Enclosed is an updated consent form.

 

1)    Document will be printed on one sheet front and back.

2)    Added medication information and log on back.

a.     The Notes /Comments / Log will give us room to annotate when we prescribe the medication.

b.     The footer states that this was added by TROOP 132.

 

If I do not hear back with issues or concerns I will bring copies to the Wednesday meeting.
