Dear Dr. Mayers:
Just following up on yesterday afternoon's conversation about the
possibility of setting up a carpool information form. Perhaps you
already have a paper form - my husband dropped our child this morning
so I'm not sure. Either way it would be good to have an electronic
version as well.
I'd be happyto transcribe any handwritten information that you may
have collected. Just let me know. I'll look for you this afternoon
when I come to pick up my daughter at 4 pm
