Tony was killed. That dude who walked into the bathroom did it.
The symbolism here is that Tony never knew what hit him. When he was
whacked, it was all just over, hence the black screen out of no where.
That's how the end comes, without warning and no memory of it.

No he didn't get whacked. By who? Phil was dead and they came to an
agreement with the little guy. He even told T You guys do what you's gotta
do. Chase focusing in on all those different people in the diner was Chase's
way of screwing with us.

entire series was mainly from Tony's perspective. It's appropriate

Yeah, I would believe that if there had been of a first person shot of Tony
looking at his family through the camera and then it went blank.....hell, I
don't know what Chase was thinking. I thought it was a great episode.
If he has to come out and say it, then he didn't tell the story
properly. I think he did.

Tony's POV, so it makes sense.
