the only one of these actually used by surrealists (except in a joking
manner) is "surrealist".
and when they're not using it as a noun...

"art" is a _process_of_interaction_ between the creator("artist") and the
"observer" via some medium (the painting or poem or performance or ...).
both are _active_ participants in art_as_experience_, although these actions
may be separated by vast distance or time.
"surrealist art" might therefore be seen as any process involving more than
one person where one or more participants is a surrealist actively exploring
the poetic dimensions of the medium of their interaction.

"surreal" and "surrealistic" are essentially popular vulgarizations of
surrealist terminology used by those that have misunderstood, or never tried
to understand, the basics of surrealism.

Dictionaries suggest that 'surreal' is a useful word, especially when
associated with the irrational quality of dreams. This may be a useful word
when not wishing to make reference to the modern surrealist movement.
BTW, there are many word sets like this that I cannot seem to figure out :
repetitive and repetitious, angry and angered, ....they really get under my
skin.
